About the Role

Age Matters is an IRT initiative that assists older people who are isolated and vulnerable navigate the complex systems of our modern world
As a Case Coordinator, you’ll be part of a dedicated and passionate team to support vulnerable and isolated older people access services under the care finder program
The successful applicant will have experience helping people navigate Aged Care and housing services and will possess a genuine desire to help vulnerable and disadvantaged older people to live independently in the broader community
This role operates on a hybrid model – split your time between working from home and across the Eurobodalla region in clients’ homes and in community locations
This position is a fixed term contract until June 2027.
Responsibilities may involve:
  • Provide specialised and intensive assistance to older people who face barriers accessing services and supports, who would otherwise fall through the cracks
  • Provide face to face outreach services for vulnerable older people in the care finder target population so they can live independent in the community
  • Effectively deliver on the care finders program outputs in accordance with the policy guidelines
  • Maintain and improve the quality of life of the clients by liaising with service providers to monitor effectiveness and appropriateness of services provided

To Be Successful You Will Have

  • Relevant tertiary qualifications in an appropriate field such social work, human services, aged care, community services or health and/or relevant experience.
  • Previous experience in a similar role providing case management to socially isolated and/or disadvantaged people, ideally interacting with Aged Care services in a local setting
  • Commitment to delivering person centred support that respects and responds to each person’s individual needs, preferences, values and life experiences, supports each person to lead in decision making, respects and facilitates optimal consumer choice.
  • A strong understanding of, and commitment to, cultural safety and trauma informed practice.
  • Extensive skills, knowledge, experience and demonstrated ability to engage with older people experiencing complex issues related to their housing, financial, health and care needs.
  • Effective interpersonal and communication skills (written and verbal) including a friendly and approachable manner appropriate for clients with complex needs
  • Experience operating effectively in a variety of settings including in a client’s home, at a centre or clinic, or in the community; proactively considering the safety and wellbeing of the client and yourself
  • The ability to work as part of a team and be able to prioritise work to meet deadlines
  • Current Drivers Licence and willingness to travel across the South Coast (Eurobodalla, Shoalhaven and Illawarra) region.
