We're looking for a Commonwealth Home Support Programme (CHSP) Coordinator to join our team.
As a CHSP Coordinator, you’ll be responsible for managing and coordinating support services including Flexible Respite, Domestic Assistance, and Home & Garden Maintenance. You’ll play a vital role in ensuring services are tailored to meet individual client needs, goals, and preferences.

About The Role
We're looking for a Commonwealth Home Support Programme (CHSP) Coordinator to join our team.
As a CHSP Coordinator, you’ll be responsible for managing and coordinating support services including Flexible Respite, Domestic Assistance, and Home & Garden Maintenance. You’ll play a vital role in ensuring services are tailored to meet individual client needs, goals, and preferences.
Responsibilities Include
- Conducting regular client reviews and home visits to assess care needs
- Identifying and addressing risks, and following up on incidents and feedback
- Liaising with families, support networks, and health professionals
- Maintaining accurate and up-to-date care plans, agreements, and documentation
- Managing complaints and feedback in a timely, professional manner
- Participating in on-call duties every sixth week (with RDO in lieu)
- Supporting other tasks as directed by management
You’re a compassionate and organised professional who brings both heart and structure to your work. You’ll thrive in this role if you have:
- Coordination or administration experience, ideally in aged or community care
- Knowledge of the aged care sector and Home Care Standards
- Strong verbal and written communication skills
- High attention to detail and commitment to accurate documentation
- Ability to prioritise tasks and manage time effectively
- A problem-solving mindset with initiative and reliability
- Empathy, patience, and a genuine desire to support others
- Dedication to delivering exceptional customer service
