Wellbeing Manager | Full-Time, 12-Month Contract | Toowoomba, QLD

We're seeking a passionate Wellbeing Manager to join our team on a full-time, 12-month contract and make a real impact across the Toowoomba Region QLD. If you're ready to create positive change every day, this could be the perfect role for you!

A Day in the Life:

• Coordinate tailored Home Care Package (HCP) plans and manage care delivery, budgets, and clinical support to ensure holistic, person-centred care at home.
• Provide nursing care one day per week, including assessments, treatments, and care planning as part of the Wellbeing Manager role.

What We Offer:

• Salary: $92,861 - $101,835 + 11.5% super (based on experience)

About You:

• MUST HAVE! Current Registered Nurse (AHPRA) with hands-on experience and a passion for supporting older adults to live independently
• Strong case management skills (ideally in community/aged care), HCP assessment experience, and a goal-oriented, holistic approach
• Excellent communication and stakeholder engagement, valid driver's licence, and willingness to travel locally
