This part-time community-based role offers the opportunity to deliver high-quality nursing care directly in clients' homes across Warwick, QLD. As client needs grow, there is potential for this role to transition into full-time.
As a Home Care Registered Nurse, you will work autonomously, managing your own schedule to ensure timely and prioritised care delivery. You’ll travel between clients' homes, providing compassionate, person-centred care that supports independence and wellbeing.

Your ability to work autonomously, exercise sound clinical judgment, and communicate effectively with clients, families, and care teams will be essential to your success.

- Conduct comprehensive assessments, develop tailored care plans, and evaluate outcomes
- Collaborate effectively with internal teams and external service providers
- Maintain accurate and timely documentation using electronic systems
- Manage your own schedule and travel between client homes to deliver prioritised care
- Work independently while being supported by a dedicated and compassionate team

- AHPRA registration as a Registered Nurse with a current Practising Certificate
- A current driver’s licence and willingness to travel between client homes
- Desirable: Postgraduate qualifications in Gerontology, Dementia, or Aged Care
- CPR and First Aid Certification
- Experience in aged care or community nursing (12 months preferred)
