About the role
We are seeking a dedicated and experienced Support Coordinator to join our team in Melbourne CBD for a permanent part-time role, working Wednesdays to Fridays. Initial training will require you to be in the office for the first 2 months. After completing training, flexible hybrid or remote working arrangements may be discussed.

As a Support Coordinator you’ll empower individuals with disabilities to navigate the NDIS and connect with the right services to achieve their goals. You'll work closely with clients, families, and service providers to coordinate supports that truly make a difference.

Responsibilities include:

  • Support clients and families in accessing and coordinating services.
  • Build relationships with internal and external stakeholders.
  • Implement supports outlined in NDIS plans.
  • Help clients develop skills to coordinate support and engage in the community.
  • Empower clients to understand and implement their plans.
  • Be responsive to client needs, especially in crises.
  • Conduct regular progress reviews and report to the NDIS.

Skills and Experience:

  • Understanding or knowledge of the NDIS
  • Prior experience in Support Coordination and/or a similar role
  • Familiarity with psychosocial disabilities (advantage)
  • Strong communication skills
  • Demonstrable problem-solving skills

Requirements

  • Valid driver’s licence
  • NDIS Worker Screening Check
  • Working with Children’s Check (WWCC)
  • NDIS Orientation Module: “Quality, Safety & You”
  • Relevant qualification in disability services, social work or similar (desirable)
  • Australian working rights (with no restrictions)
