Our Client:

Our client has an excellent reputation for delivering exceptional care and service to their residents in a positive and compassionate way. As a highly valued Clinical Care Coordinator working in a medium sized facility, you will join a team who are renowned for providing high standards of care to residents and their families. This role is supported by an amazing Clinical Nurse Manager and Facility Manager who will value your contribution.

Key Responsibilities:

  • Provide clinical leadership and supervision of care teams including Nurses, ENs and Personal Carers
  • Develop care plans in consultation with the resident, family/carer and other members of the care team
  • Promote a culture of service delivery that aims to improve the lifestyle experiences of residents
  • Support the Clinical Nurse Manager and work collaboratively to enhance the care and services
  • Ensure accurate information is recorded and in accordance with all organisational and legislative requirements

The Successful Candidate:

  • CCC experience or suit Senior RN/Lead RN in Charge or Clinical Nurse looking for the next step
  • Current AHPRA registration and clinical experience in aged care working in a residential setting
  • Support and mentor staff in a clinical care setting
  • Knowledge of current aged care standards and audit processes
  • Knowledge of AN-ACC and resident care
  • Highly personable with the ability to support staff and build exceptional relationships
  • Current influenza and COVID vaccinations
  • NDIS clearance and National Police Clearance (within 3 months)
