Housing Support Practitioner

Join our client as a Housing Support Practitioner, providing vital support to veterans facing homelessness or at risk of it. You'll deliver trauma-informed, person-centred support to help veterans and their families access stable, safe housing. Using a strengths-based approach, you’ll guide clients through housing options, case management, and tenancy sustainment strategies.

Key Responsibilities:

• Provide comprehensive homelessness support to veterans and their families.
• Coordinate personalized housing plans, engaging clients in goal setting.
• Case manage 15-20 clients, helping with housing applications and securing housing options.
• Advocate for veterans to access community and specialist services.
• Maintain accurate documentation and collaborate with internal and external stakeholders.
  
  

Qualifications & Experience:

• Bachelor’s degree in Social Work, Human Services, or a related field.
• Experience in social work, housing, or homelessness services is desirable.
• Strong knowledge of Queensland tenancy legislation and homelessness frameworks.
• Familiarity with veteran and ADF community needs.
• Ability to work flexibly and collaboratively with diverse teams.
