This role is for HomeMade - a related entity of Mable
HomeMade is a related entity to Mable and operate as a completely separate business, however both have a shared vision for self-management.
We are a health-tech start-up established in 2020, focusing on transforming home care in Australia through our creative approach which is designed to support people in achieving their goals whilst living at home.
Leveraging on our technology platform, we are an approved provider that specialise in self-management. We partner with our customers to create unique support plans that meet their needs and optimise their funds, making the most out of their Home Care Package (HCP). Our approach is simple and keeps people in control of their package, funding, and support.

You’ll work collaboratively and proactively through telehealth to help customers stay safe and independent at home. You’ll triage clinical concerns with care, confidence, and clarity, providing expert clinical input to guide decisions. Most importantly, you’ll be part of a forward thinking team reshaping aged care that genuinely values your knowledge and the impact you make every day!

Role Responsibilities
Effectively deliver on all requirements of the Clinical Roster including:
- Monitor and triage the Clinical Inbox and wellbeing notes
- Complete clinical assessments and reviews (e.g., VATs, CNAs)
- Support customers and families with informed decision-making
- Guide palliative care and advance care planning
- Respond to incidents and clinical escalations
- Contribute to continuous improvement and clinical quality initiatives
- Provide clinical input across our broader team and provider network
- Align your practice with the Aged Care Act, Quality Standards, and our Clinical Governance Framework
- AHPRA Registration as a Registered Nurse
- Experience in Home and Community Care, ideally with aged care clients
- Strong clinical decision-making and case management skills
- A warm, human approach and genuine passion for empowering others
- Excellent communication and customer service skills
- First Aid certification, full driver’s license and access to your own vehicle
- Bonus points if you know your way around self-managed or consumer-directed care
