ABOUT THE ROLE
- Manage the day-to-day operational and reporting requirements of the Commonwealth Home Support and Care Finder programs.
- Oversee the planning, facilitation, and growth of the Social Support Group activities.
- Ensure team members provide a consistent, effective and compliant case management structure to ensure clients achieve positive outcomes.
- Lead the internal Quality Advisory Group to ensure the Aged Care Service have met the required standards.
- Manage client relationships and the resolution of all incidents/complaints raised by clients, families or other stakeholders and ensure a review of practice to ensure improvement.
ABOUT YOU
- Hold a relevant tertiary or associated diploma in Aged Care/Community Services/Nursing (RN highly desirable)/Management or equivalent relevant experience.
- Passionate about supporting people by providing quality care
- Experience in leadership and providing case management in home, community care or aged care
- Sound understanding of Aged Care Quality Standards, Strengthening Standards and Aged Care Reform Agenda
- Strong commitment to compliance including the clinical governance framework
- Excellent team management, communication and the ability to work with a broad range of stakeholders
