The Salary range is $51.75 to $53.84 per hour. Hourly rate based on experience.
Location: Located in coastal Bundaberg, Gracehaven offers aged care and retirement living in a vibrant, community-focused town rich in nature, heritage, and lifestyle.

What You’ll Do
As Care Coordinator, you’ll lead the delivery of high-quality clinical care for residents with complex needs. In this pivotal leadership role, you will:
Key Responsibilities
- Oversee resident assessments and develop tailored care plans
- Coordinate clinical documentation and compliance reporting (AN-ACC)
- Lead and mentor a team of RNs, care staff, and volunteers
- Collaborate with external providers to ensure seamless care delivery
- Monitor and improve clinical programs and quality indicators
- Champion a culture of safety, wellbeing, and continuous improvement
About You
As a Care Coordinator, you’ll play a pivotal role in leading clinical care delivery for residents with complex needs. You’ll oversee assessments, care planning, and compliance, while mentoring a team of Registered Nurses, care staff, and volunteers. Your leadership will ensure our residents receive holistic, person-centred care aligned with our Positive Wellbeing Model.
This is a key leadership position reporting to the Service Manager, with direct responsibility for clinical staff and indirect influence across the broader care team.
Requirements:
- AHPRA Registration as a Registered Nurse (RN) with a current Practicing Certificate
- Minimum 3 years’ post-graduate experience in aged care or acute clinical settings
- Proven leadership and workforce management skills
- Strong clinical assessment, care planning, and documentation capabilities
- Intermediate computer literacy (Word, Excel, Outlook, PowerPoint)
- A commitment to quality, safety, and continuous improvement
- Current National Police Check – or willingness to acquire
- Current First Aid & CPR certification – or willingness to acquire
- A Valid driver’s licence
Desirable:
- Postgraduate qualifications in Gerontology, Dementia, or Aged Care
- Experience with mobile clinical documentation systems
