North and West Remote Health (NWRH) is currently seeking an adaptable Journey Coordinator within the Stepped Care service system to join our team.


The Journey Coordinators are dedicated positions within the Stepped Care service system that operates as an independent partner in a person’s care journey and may be a standalone service, delivered within a Front Door service, or delivered as an adjunct to another Stepped Care service offering. The Journey Coordinators will ensure that individuals are placed at the centre of their own health and wellbeing, and experience improved care and mental health.


Your primary responsibility will be to deliver high-quality support and services by improving outcomes for Stepped Care participants. Continue to build psycho-social education, support to access self-management approaches including digital mental health services and group programs amongst facilitating the Stepped Care model of ensure the quality, continuity and coordination of care between service providers.


This role is offered on a full-time basis. This role is based in Cairns and has outreach travel.

These principles provide the foundation for the future system and will guide service delivery.


About you


We are looking for individuals who have the following qualities and experience:



  • Ability to establish, develop and maintain multiple productive working relationships and networks

  • Experience identifying and delivering partnership and collaboration opportunities that enhance consumer outcomes and improve the service system

  • An understanding of mental health challenges and how they inform an individual’s experience

  • Ability to develop and maintain a genuine relationship with individuals accessing Stepped Care

  • Ability to adopt a recovery-focused, person centered, strengths based and trauma informed approach

  • At minimum a Certificate IV in any Mental Health related qualifications. Only those with suitable qualifications will be considered.
