About the role

Mercy Community are on the search to find an experienced Care Partner to join our Community Aged Care Services team based in Brisbane on a Permanent Full-time basis. This role will be based at our Nudgee office. Travel to Southside of Brisbane will be required.

The Care Partner is the designated person under Community Care, responsible for delivering care management that enables older people to live safely and independently at home. Working within a rights-based, person-centred framework, the Care Partner collaborates with participants, their supporters, and interdisciplinary teams to coordinate tailored, responsive and inclusive support. Salary commensurate with experience plus superannuation plus salary packaging available

This role involves but not limited to -

  • Collaborate with participants to develop individualised care plans within required funding timeframes
  • Incorporate the participant’s goals, and environmental requirements within the participant care plan and delivery
  • Establish and update care plan and funding requirements, ensuring participants understand their available services and financial responsibilities
  • Review care notes and service delivery data

About you

As the successful applicant, you will possess -

  • Diploma-level qualification in Community Services, Aged Care, Case Management, or equivalent
  • Demonstrated experience in care coordination, case management or similar roles in aged care, health, or disability services
  • Strong communication, empathy and cultural competence.
  • Skilled in using care management or CRM systems
