The Derbarl Yerrigan Health Service (DYHS) was established in 1974 and is the largest Aboriginal community-controlled health service provider in Western Australia, providing care to over 20,000 Aboriginal patients across four clinics in East Perth, Mirrabooka, Maddington and Midland. Our staff enable the delivery of culturally responsive primary health services, focusing on prevention, early intervention, comprehensive care and care coordination including chronic disease management.

This position is responsible for coordinating and timely managing recalls and reminders for clients.
Key Responsibilities
- Coordinate DYHS’s recall and reminder system to ensure timely management of recalls.
- Work with the Medical Director and Clinical Governance Manager to ensure urgent recalls are appropriately managed.
- Delegate recalls to all four clinics (East Perth, Maddington, Midland and Mirrabooka) for action and follow-up any outstanding items.
- Ensure the doctors receive a recall list and outstanding referral list every month to address.
- Work with the clinic managers to ensure each site has a dedicated recall practitioner each day.
- Work with the Medical Director to ensure the recall and reminder policy is relevant and updated.
- Other clinical duties within the scope of an AHP, as directed by the Clinical Governance manager.
- Adhere to all Derbarl policies and procedures including requirements of the Workplace Health and Safety Legislation and infection control guidelines.
- Plan and Coordinate transport for urgent recalls in collaboration with Clinic Managers as needed.
- Qualifications:
- Current Registration with the Australian Health Practitioner Regulation Agency as an Aboriginal Health Practitioner.
- Certificate IV or Diploma in Aboriginal Health.
- Experience, Skills and Abilities:
- Demonstrated experience in coordinating and supporting the team.
- Knowledge of current health issues, impacting on Aboriginal Health.
- Understanding the principles of public health including health education, health promotions and disease prevention.
- Well-developed written and verbal communication skills.
- Knowledge and understanding of the holistic approach to health service provision.
- Experience working in a multi-disciplinary team environment.
- Demonstrated ability to maintain confidentiality and security of records and Information.
- Knowledge of Work Health & Safety, Industrial Relations and Equal Employment Opportunity Legislation.
- Other Requirements:
- Aboriginal and/or Torres Strait Islander pursuant to Section 50D of the Equal Opportunity Act.
- Current Working with Children Check.
- Current satisfactory National Police Clearance.
- Valid WA Drivers’ License and willingness to travel as required.
- Computer literacy skills.
- Provision of the minimum identity proofing requirements
- Successful Criminal Record Screening Clearance
- Successful Pre-Employment Health Assessment
- Successful Working with Children Check
- Evidence of a current C or C-A Class driver’s licence or other specialised licence class
