About the Role
This unique opportunity offers the chance to support vulnerable patients from the comfort of their homes. You'll grow your healthcare knowledge while making a genuine impact. Ideal for someone empathetic, tech savvy, and ready to work in a collaborative team environment.
In this role, you’ll support patients with complex health needs remotely, using regular phone calls to build rapport and improve outcomes. You’ll work closely with Health Coaches and other care providers to identify early signs of deterioration and help reduce unnecessary hospital visits. This is a chance to contribute to a proactive healthcare model while developing valuable communication and support skills.
Key Responsibilities
- Call patients regularly to monitor well being
- Report health changes and escalate as needed
- Schedule and coordinate appointments and referrals
- Provide a respectful, inclusive phone experience
- Manage clerical tasks and documentation
- Work independently and with the broader care team
Skills & Experience
- Customer service or call centre background
- Comfortable with Microsoft Office and phone systems
- Able to work autonomously and adapt quickly
- Experience in aged care, disability or emergency health (advantageous)
- Cultural awareness and non-judgemental communication
