About the Role
Grampians Health Ballarat is seeking a motivated and enthusiastic clinician to join our Emergency Department Care Coordination (EDCC) team.
This is a unique opportunity to work in a dynamic and fast-paced environment where you’ll facilitate timely access to an integrated system of community-based services. EDCC staff play a vital role in supporting person-centred care, safe discharge, and service continuity across hospital and community settings.
You will be supported with 2 days of paid supernumerary EDCC training alongside an experienced team member.
Key Responsibilities
- Facilitate timely and safe discharge from the Emergency Department
- Coordinate referrals to community-based services
- Provide a primary point of access for external service providers
- Promote continuity of care between acute and community settings
- Collaborate effectively with ED staff and the wider multidisciplinary team
- Support patients and carers to participate in care planning and decisions
Skills & Experience
- Registered Nurse (Division 1) with AHPRA (desirable)
- Postgraduate qualification in a relevant area (highly desirable)
- Demonstrated experience in acute and community care
- Strong skills in assessment, discharge planning and interdisciplinary teamwork
- Excellent interpersonal and communication skills
- Ability to work both independently and collaboratively in a fast-paced setting
- Willingness to pick up additional EDCC shifts when available
