What your day will look like....

Your day will be dynamic and fulfilling as you engage with patients and their families, providing support and advocacy through home visits, case management, and follow-up care. You will coordinate clinical care, develop patient case plans, and conduct health promotion and education sessions within the community. Collaborating with other health professionals and external agencies, you will ensure the delivery of evidence-based care. Additionally, you will maintain accurate documentation, monitor Indigenous patient identification, and participate in cultural events, promoting awareness and education. Your role will also involve mentoring other health workers and contributing to the continuous improvement of health services for the Indigenous community.
