About The Opportunity
We are seeking a compassionate, enthusiastic and experienced care advisor with an allied health background to join our dynamic team of care advisors. In this role you will be responsible for managing a case load of Home Care Package/support at home clients and ensuring he delivery of high-quality, person-centred care. You will require experience in assessment and a proactive approach to care planning, coordinating of services as well as an understanding of the needs of elders with complex health conditions who are from a CALD community.
The Day-to-day Responsibilities
- Establish and nurture effective relationships with clients to develop and maintain high-quality, individualised care plans.
- Manage a portfolio of clients, overseeing their care needs and Home Care Package (HCP) funding.
- Work within Fronditha Care's client directed care model to ensure service delivery aligns with client's preferences and needs.
- Ensure all services comply with the HCP Operation Manual, the Aged Care Act and relevant legislation.
- Collaborate with clients to develop implement, monitor and review personalised care plans, respecting their autonomy and working within the Aged Care Quality Standards
- Ensure that work practices are up to date with industry trends.
- Uphold a commitment to continuous improvement and strive for the achievement of excellence in service delivery.
- Work with the Client Services team to ensure consistent and reliable delivery of services.
- Maintain awareness of and adhere to OH&S requirements.
- Oversee quality standards in compliance with organisational policies and procedures.
What Success Looks Like For This Role
- Registered Nurse (RN), Allied Health Professional, or an equivalent qualification in aged care or health services.
- Minimum 2 years’ work experience in Aged Care.
- Strong understanding of aged care standards, regulations and funding models.
- Excellent communication, management and team leadership skills.
- Understanding of Aged Care and Dementia Care.
- High degree of computer literacy.
- Demonstrated experience in managing care packages in accordance to contractual requirements and government programs and guidelines.
- A current driver’s licence with access to own registered vehicle.
- Greek communication skills desirable.
