Physiotherapist (Tcp, Slow Stream Rehabilitation) - Geelong, Vic

We are seeking a Physiotherapist to join a dynamic allied health provider in Geelong, VIC. You will deliver timely, evidence-based physiotherapy within a transition care and slow stream rehabilitation setting, helping older people regain independence and return home with confidence. You will work closely with a supportive Team Leader and a collaborative multidisciplinary team.

  • Deliver high quality, timely, evidence-based physiotherapy within a rehabilitation setting
  • Work within legislative, regulatory and scope of practice requirements
  • Conduct clinical assessments and develop and maintain care plans and clinical goals
  • Support clients to build strength, mobility and independence, and to transition safely home
  • Communicate professionally with all stakeholders and apply a person-centred approach
  • Keep documentation to a high standard, in line with company, site and medico-legal requirements
  • Seek advice and clinical support from your Team Leader when needed
  • Prepare reports as required
  • Liaise with partner organisations in line with contractual expectations
  • Follow company policies and practices
  • Attend team meetings as required

About The Candidate

  • Tertiary qualification in Physiotherapy
  • Current AHPRA registration
  • Experience in rehabilitation, transition care or working with older people (desirable)
  • Demonstrated competence delivering therapy across a wide range of clinical conditions and service settings
  • Experience meeting key performance measures
  • Experience managing others to achieve key performance measures (desirable)
  • Valid driver's licence
