Care Coordinator - MHAOD Connect

About the role
The MHAOD Connect service provides timely and effective community based post-discharge support for up to six weeks for individuals who present to Sunshine Hospital Emergency Department or Footscray Hospital Emergency Department with Mental Health and/or AOD care concerns.
The role is part of Western Health Drug Health Services and works closely with Emergency Department based AOD Clinical Nurse Consultants, Addictions Registrars, Addictions Specialists and AOD Nurse Practitioners as well as Emergency Department staff, including the Emergency Mental Health service.
The Care Coordinator works as part of the multidisciplinary MHAOD Connect team providing AOD related assessments, counselling interventions and referrals.
Key responsibilities
- Coordination of the intake functions of the service, including undertaking initial needs assessments with consumers who have been newly referred to the service.
- Provision of social work support and case coordination to consumers accessing the service.
About you
- Bachelor/Master of Social Work degree and eligibility for AASW membership.
- Minimum 3 years professional social work experience, preferably in an AOD, Mental Health or hospital/health care setting.
- Demonstrates a sound understanding of a broad range of theoretical frameworks, including trauma informed care, recovery-oriented practice, human rights, and social justice principles.
- Highly developed interpersonal, written and electronic communication skills.
