About the role
An exciting opportunity is available for an experienced Occupational Therapist to join the Intensive Support Program (ISP) within Western Health’s Outer Community Mental Health and Wellbeing Service.
Based at Melton, you will work within a small, cohesive multidisciplinary team providing intensive, recovery-oriented support to consumers with complex mental health and psychosocial needs across Melton and Brimbank.
This role combines case management responsibilities with occupational therapy-specific assessment, intervention and consultation. You will manage an allocated caseload while using your occupational therapy expertise to understand how mental health affects consumers’ daily functioning, routines, relationships, meaningful activities and participation in the community.
Key responsibilities
You will:
- Provide recovery-oriented case management to an allocated caseload of consumers.
- Undertake mental health, risk, psychosocial and occupational therapy assessments.
- Develop and deliver individualised interventions that promote independence, functional recovery and community participation.
- Coordinate care in partnership with consumers, families, carers and internal and external service providers.
- Provide occupational therapy consultation and discipline-specific expertise to the ISP team and broader service.
- Contribute to service development, quality improvement and a positive multidisciplinary team culture.
- Maintain high standards of clinical documentation and work within relevant legislative, professional and organisational requirements.
About you
To be successful in this role, you will have:
- An approved bachelor’s or master’s degree in occupational therapy, or an equivalent qualification recognised by the Occupational Therapy Board of Australia.
- Current general registration as an Occupational Therapist with the Occupational Therapy Board of Australia through AHPRA.
- A minimum of two years’ experience working in community mental health or a similar clinical setting with consumers who have complex needs.
- Knowledge of recovery-oriented practice and the Mental Health and Wellbeing Act 2022.
- Strong assessment, care coordination, communication and organisational skills.
- The ability to work effectively within a multidisciplinary team.
- A current Victorian driver’s licence and the capacity to drive work vehicles.
