About the Role

The Ongoing Support Practitioner helps participants with disability, injury or health conditions achieve and sustain meaningful employment. The role provides person-centred support, identifies barriers and development needs, and delivers practical solutions to workplace challenges. By building strong relationships with participants and employers, they support long-term employment outcomes while delivering quality service and achieving key performance targets.

Reporting to the Regional Manager key responsibilities of this role will include:

  • Build strong relationships with participants approaching Ongoing Support phase, identifying their barriers, skills, abilities and workplace support needs.
  • Coordinate Ongoing Support Assessments (OSA), preparing participants and Employment Consultants / Practitioners and ensuring all required administration is completed.
  • Maintain regular contact with participants and employers to monitor wellbeing, workplace needs and provide ongoing mentoring and support.
  • Develop tailored interventions and practical solutions to address workplace challenges and support participants to maintain sustainable employment.
  • Build employer partnerships and identify opportunities, including job carving, to create suitable and sustainable employment outcomes.
  • Manage compliance, documentation and claims, ensuring participant obligations, contact records, supporting evidence and claims are completed accurately and on time.
  • Plan and manage service delivery effectively across multiple locations, ensuring all activities meet contractual, guideline and service requirements.

About You

The ideal applicant we are seeking will have:

  • Have a strong working knowledge of the IEA contract as well as experience working with people with disability, with lived experience or qualifications in employment or disability services highly regarded.
  • Strong communication and relationship-building skills, with the ability to engage effectively with participants, employers and a diverse range of stakeholders.
  • Experience in person-centred case management, including barrier management, mentoring, coaching and supporting sustainable employment outcomes.
  • Strong problem-solving and conflict resolution skills, with the ability to develop practical and creative workplace interventions.
  • Resilient, adaptable and culturally competent, with the ability to work effectively with people from diverse backgrounds and collaborate within a remote team.
  • Strong administrative and digital capability, with sound computer literacy, attention to compliance and knowledge of relevant disability employment standards and guidelines.
  • Commitment to policies, quality, safety and continuous development, including compliance with QMS, information security, WHS, regulatory and contractual requirements.
  • A valid unrestricted driver licence is required.
