$105,000–$110,000 + super + salary packaging

Dementia and Aged Care Services (DACS) is seeking an experienced Client Relations Manager to oversee a portfolio of clients receiving services through the Support at Home program.

You will build trusted relationships with clients and families, coordinate quality and responsive services, and manage client budgets to ensure funding is used effectively. Your work will help older people achieve their goals, respond to changing needs and continue living well at home.

Key responsibilities

  • Manage a portfolio of approximately 50–60 Support at Home clients.
  • Develop and review goal-focused care and service plans.
  • Coordinate services that promote wellness, independence, reablement and social connection.
  • Manage client budgets, expenditure and service utilisation.
  • Help clients understand their funding, fees and service options.
  • Build strong relationships with clients, families and service providers.
  • Monitor service quality and respond promptly to changing needs, risks or concerns.
  • Maintain accurate records and meet quality, compliance and reporting requirements.
  • Strengthen local referral networks and support service growth.

About you

You are an experienced aged care or community services professional who combines strong client relationships with sound financial and operational judgement.

You will bring

  • Experience in Support at Home, Home Care Packages or community aged care.
  • A background in care management, case management or care coordination.
  • Confidence managing client budgets and service utilisation.
  • Knowledge of person-centred care, wellness and reablement.
  • Strong communication, organisation and problem-solving skills.
  • A current driver’s licence and access to a reliable vehicle.
