Assist Ability Australia is looking for a passionate and experienced Support Coordinator to join our growing team!

We are seeking a Part-Time Support Coordinator who shares our commitment to making a meaningful difference in the lives of NDIS participants.
About the Role
As a Support Coordinator, you will play a key role in assisting NDIS participants to understand and implement their plans, connect with appropriate services, build their capacity, and work towards achieving their personal goals.
Your responsibilities will include
Providing high-quality Support Coordination to a diverse range of NDIS participants
Building strong relationships with participants, families, carers and key stakeholders
Coordinating supports across multiple providers to ensure effective service delivery
Supporting participants to navigate the NDIS and exercise choice and control
Monitoring participant plans, funding utilisation and service outcomes
Preparing reports, progress updates and maintaining accurate participant records
Collaborating with allied health professionals, service providers and community organisations
Advocating for participant needs while promoting independence and self-advocacy
About You
Previous experience in Support Coordination or Case Management
Strong understanding of the NDIS and participant-centred practice
Excellent communication and stakeholder engagement skills
Strong organisational skills and the ability to manage competing priorities
Ability to work independently while contributing positively to a collaborative team environment
Strong report writing and documentation skills
Mandatory Requirements
Current NDIS Worker Screening Check
Working With Children Check
National Police Check (within the last 12months)
NDIS Worker Orientation Module completion
Current First Aid & CPR (or willingness to obtain)
Valid Australian Driver Licence and reliable vehicle
