The Service

Hospital in the Home (HITH) is Cabrini’s acute outreach service, providing hospital-level treatment to patients in the comfort of their own homes. The service also delivers care to Post Acute Care and Chemotherapy patients under our Cabrini at Home program.

The Role

We are seeking an experienced Hospital in the Home (HITH) Nurse to join our team on a full-time or part-time basis, with flexible FTE options available for the right candidate.

Working across a rotating 7-day roster, including some PM and on-call shifts, you will deliver high-quality acute care to patients in their homes. This senior clinical role offers the opportunity to work both independently and as part of a multidisciplinary team, partnering with physicians, specialists and allied health professionals to provide safe, evidence-based, patient-centred care.

Key Responsibilities

  • Develop and implement comprehensive care plans tailored to each patient’s needs

  • Monitor patient progress and ensure continuity of care

  • Conduct clinical assessments, administer treatments, and manage patient care

  • Provide education to patients and families on health management and recovery

About you

To be successful in this role, you will possess:

  • Current AHPRA registration as a Registered Nurse

  • Proven acute and community nursing experience

  • Strong clinical skills in wound care, IV therapy, and CVC management

  • Ability to work independently and respond to clinical deterioration

  • Excellent communication and documentation skills

  • Commitment to quality improvement and ongoing professional development

  • A valid driver’s licence and Working with Children Check
