About the Role
As a Care Partner, you will coordinate and deliver high-quality aged care services across Townsville and surrounding areas. You will manage client referrals, complete care assessments, develop and review care plans, and conduct home visits to ensure clients receive personalised support.
You will build strong relationships with clients, families, and stakeholders while helping to improve day-to-day processes and ensure a positive client experience.
This is a permanent full-time role based in Townsville, with regular travel throughout the region for home visits. It's a great opportunity for a motivated professional looking to grow their career in aged care and make a meaningful difference.
Key Responsibilities
- Conduct client assessments and develop care plans with clients, representatives, and stakeholders
- Update care plans regularly and conduct home visits
- Monitor clinical risks and client needs, ensuring appropriate referral pathways are in place
- Work collaboratively with internal teams, including Rostering and Practice Leads, to meet client needs
- Develop strategies and/or broker services to address the identified needs of Aboriginal and Torres Strait Islander clients in consultation with families and communities
- Represent Life Without Barriers at external forums and meetings, promoting the organisation within the community
Skills & Experience
- Demonstrated Aged Care case management experience
- Understanding of Aged Care funding and service delivery, specifically Support at Home
- Confidence in utilising a variety of technology including mobile devices and computers
- Proficiency with Microsoft Office, Outlook and database systems
- Ability to effectively build relationships with diverse stakeholders
- Strong communication skills
- Ability to multi-task and work autonomously in a fast-paced environment
- A current unrestricted Australian Drivers Licence, reliable vehicle and a willingness to travel
