Hospital in the Home Coordinator
Epworth at Home | Full Time | Richmond, Freemasons & Eastern
Join Epworth's expanding Hospital in the Home (HITH) service and play a pivotal role in delivering acute hospital-level care in patients' homes. As the Hospital in the Home Coordinator, you'll oversee the patient journey into HITH and Chemotherapy in the Home (CITH), ensuring seamless transitions, exceptional patient outcomes and strong clinical leadership across the service.
Epworth HITH was the first private sector Hospital in the Home service established in Australia and continues to lead the delivery of innovative acute care in the home environment. Working closely with operational and clinical leaders across Epworth at Home, you'll help drive service growth, patient access and revenue optimisation, supported by your understanding of private health funding and health fund processes.
In this role you will
- Coordinate patient transitions into HITH and CITH services
- Provide clinical leadership, education and advice to nursing and multidisciplinary teams
- Build strong relationships with internal and external referrers
- Support service development, quality improvement and integrated models of care
- Ensure patients receive safe, high-quality, person-centred care throughout their journey
- Contribute to revenue optimisation through effective management of health fund processes and service access pathways
Qualifications
To be successful you will have
- Current AHPRA registration as a Registered Nurse
- Previous Hospital in the Home (HITH) nursing experience (essential)
- Demonstrated clinical expertise in coordinating complex patient care
- Knowledge of the private health environment and health fund arrangements
- Strong leadership, communication and stakeholder engagement skills
- Ability to work autonomously while building collaborative relationships across multidisciplinary teams
- Current Victorian driver's licence
- Postgraduate qualifications and management experience are desirable

The annual salary for a full time Clinical Consultant (CCA1-CCC2) – AUD $121,889 - $137,918 + super + entitlements depending on registration type, year level and position (prorated for part-time)
