We are seeking an experienced and passionate Clinical Lead, Community Health Nursing to provide senior clinical leadership across our Community Health Nursing, Post Acute Care, Support at Home, and emerging community-based programs.

About The Role
Reporting to the Program Manager, Primary Care & GP Services, you will provide advanced clinical leadership and governance oversight across multiple community-based nursing programs. You will play a pivotal role in ensuring safe, evidence-based, person-centred care while supporting nurses and multidisciplinary teams to achieve exceptional client outcomes.
A key focus of this role will be leading the implementation and clinical oversight of new service models, including chronic disease care pathways and Support at Home clinical advisory services, positioning BHN for future growth and funding opportunities.
About You
You are an experienced nursing leader with a strong background across both acute and community healthcare settings. You are collaborative, outcome-focused and passionate about improving care for people living in the community.
You Will Bring
- Relevant tertiary qualification and background in nursing with current registration or eligibility under relevant professional bodies.
- Significant experience across acute and community health environments.
- Strong clinical expertise in complex assessment, chronic disease management and community-based care.
- Proven leadership experience in clinical governance, supervision and multidisciplinary team coordination.
- Experience developing, implementing and evaluating clinical programs.
- Understanding of community health funding models including Post Acute Care, Support at Home and chronic disease care pathways.
- Experience in risk management, care planning and case review processes.
- Commitment to person-centred, outcomes-focused care.
