  • Location: Melbourne
  • Type: Office | Hybrid Working - Office + Remote
  • Contract: Part Time | Maximum Term (30.4 hours per week) contract ending on 31 August 2027
  • Hours: 30.4 hours per week with flexible work pattern available
  • Salary: Social & Community Services Level 4 | $46.70 per hour + Super

About The Team

You'll be part of a close-knit team that values collaboration, compassion and purpose. Working across our South Melbourne office and Lakehouse Residences, you'll partner closely with the Program Coordinator to build meaningful relationships with residents and support them to achieve lasting housing and wellbeing outcomes.

About This Opportunity

As a Case Manager, you will provide holistic case management to women aged 40+ living at Lakehouse Residences, a transitional housing program in South Melbourne. The program supports women to settle and stabilise, build independence and economic participation, and successfully transition into long-term housing in the community.

In This Role You Will

  • Provide person-centred case management, including assessment, support planning and goal setting tailored to individual needs and aspirations.
  • Build trusted relationships with residents, offering practical assistance and encouragement as they navigate complex life circumstances.
  • Connect residents with housing, health, employment and community services, coordinating referrals and working collaboratively with external agencies to achieve positive outcomes.
  • Support women to develop the skills, confidence and resources needed to transition successfully into long-term housing.
  • Maintain accurate records and case notes, contribute to reporting requirements and support continuous improvement activities.

You’ll Be a Great Fit If You

Your experience may include

  • Relevant tertiary qualifications in Social Work, Social Science or a related discipline, or demonstrated experience in a similar role.
  • Experience providing case management to people experiencing homelessness or at risk of homelessness.
  • A strong understanding of the challenges facing marginalised and vulnerable people, and the ability to deliver effective, client-centred support.
  • Experience working collaboratively with housing, community, health and government services to achieve positive outcomes.
  • Proficiency in Microsoft Office and client management systems, with the ability to maintain accurate records and documentation.
