Northern Health are seeking applications from motivated and suitably experienced Digital Navigators to join the Clinical Leadership, Effectiveness and Outcomes (CLEO) team.

Northern Health is the major provider of acute, maternity, sub-acute, mental health, specialist, community and home-based services in Melbourne’s rapidly growing outer north.

Northern Hospital Epping, Broadmeadows Hospital, Bundoora Centre, Craigieburn and Mernda Community Hospitals, and Kilmore District Hospital. In addition, Mental Health Services are provided at Epping, Broadmeadows, Jacana, Preston, Mill Park and Coburg.

About the Role

This is an exciting opportunity to support the delivery of digitally enabled care within the Clinical Leadership, Effectiveness and Outcomes (CLEO) team. Through digital care pathways and programs, this work aims to improve health outcomes and equity by helping patients access high-quality, person-centred care. A key focus of the role is reducing barriers to care by supporting patients to confidently engage with digital tools and services, particularly those who may face challenges related to distance, mobility, or digital literacy.

As a Digital Navigator you will be at the frontline of this innovative model of care. You will:

  • Support patients and carers to access and navigate the My Health@Northern App and digital care pathways.
  • Provide practical assistance and troubleshooting to enhance digital confidence and engagement.
  • Deliver administrative support including appointment scheduling, referral coordination, patient record management, correspondence, and equipment distribution.
  • Work as part of a multidisciplinary team alongside allied health, nursing, medical and CLEO staff to ensure seamless, person-centred care.
  • Help reduce barriers to care by supporting patients with low digital literacy, culturally and linguistically diverse backgrounds, and those experiencing inequities in access.

This role combines digital health, patient engagement, and administrative expertise to enable safe, efficient, and innovative healthcare delivery.

To be successful in this role you will have:

  • Strong digital skills and confidence with apps, devices, and systems, and the ability to assist others in building digital literacy.
  • Excellent interpersonal skills with empathy, patience, and cultural sensitivity in supporting patients from diverse backgrounds.
  • Demonstrated administrative experience, with attention to detail in scheduling, records management, and correspondence.
  • Commitment to equity, inclusion, and safe, person-centred care.
  • The ability to work flexibly, adapt to new systems, and contribute to a culture of continuous improvement and innovation.
