About the Role

We are seeking an experienced Care Partner to lead the establishment and delivery of our Aged Care services under the Support at Home Program on a permanent basis, 16 hours per week.

Reporting to the Operations Manager, this role will see you take ownership of client onboarding, care planning, and ongoing case management, ensuring services are delivered in a person-centred, compliant, and high-quality manner.

You will also play a key role in developing and supporting our workforce, ensuring staff are equipped with the knowledge and skills needed to provide exceptional care.

Key Responsibilities

  • Establish and grow Supported Home's Aged Care services
  • Attract, onboard, and manage clients within the program
  • Develop, implement, and review individualised care plans aligned to client goals
  • Provide ongoing case management, monitoring, and service coordination
  • Ensure compliance with Aged Care Quality Standards
  • Build and maintain strong relationships with clients, families, and stakeholders
  • Collaborate with internal teams to upskill staff in person-centred care practices
  • Monitor service delivery, ensuring quality, consistency, and positive client outcomes

About You

You are a passionate aged care professional with strong case management experience and a drive to improve client outcomes.

To be successful, you will have

  • Proven experience in aged care assessment, care planning, and coordination
  • Previous experience in case management or care planning roles
  • Strong knowledge of Aged Care Quality Standards and Home Care funding
  • Experience supporting clients living with dementia
  • Demonstrated ability to lead, mentor, and upskill teams
  • Excellent communication and relationship-building skills

Desirable

  • Qualification in aged care, nursing, disability, case management, or a related field

Requirements

  • Current NDIS Worker Screening Check (or ability to obtain)
  • Current Driver’s Licence and access to a comprehensively insured vehicle
  • Current First Aid & CPR certification
