Allied Health at Cabrini is a rapidly expanding and highly integrated service comprising twelve professional disciplines. The department plays a vital role in delivering high-quality, patient-centred care across the organisation.

The Role

We are seeking a highly motivated Grade 3 Occupational Therapist to join our progressive and supportive Allied Health team in this newly created Allied Health Care Navigator role. This is a full-time position, based within the Occupational Therapy team and working closely with Emergency Department (ED)/ Expedited Care Unit (ECU) and other medical wards.

The AH Care Navigator is a clinical expert within their profession (Occupational Therapy) and will play a key role in facilitating the implementation of the Low Back Pain and Dizziness Clinical Pathways. The role focuses on enabling early Allied Health referral, identifying patient discharge needs, and coordinating referrals to relevant specialties, including medical, nursing, and community services.

Working closely with ED/ECU, the AH Care Navigator will also support medical length of stay (LOS) initiatives across admitted wards by facilitating early Allied Health engagement and proactive discharge planning. This role is integral to supporting timely, coordinated care and reducing hospital length of stay.

As this is a newly established position, the successful applicant will have the opportunity to help shape and refine the role, contributing to organisation-wide LOS initiatives and improving patient flow and outcomes through well-coordinated care.

The successful applicant will have sound clinical experience in Occupational Therapy within an acute setting, along with exposure to sub-acute care and a strong understanding of community services to support safe and effective discharge planning.

Relevant Qualifications & Key Capabilities

  • Recognised degree in Occupational Therapy

  • Greater than 7 years’ clinical experience

  • Experience in acute medical care, emergency department (ED), general medicine, aged care, and/or a care coordinator role within an acute hospital

  • Ability to work effectively within a multidisciplinary team, both independently and in a structured environment

  • Commitment to high-quality clinical care and well-developed clinical skills

  • Strong interpersonal and communication skills (written and verbal) with a strong patient focus

  • Sound awareness of discharge planning needs and available services/resources

  • Experience in a care coordination role is highly desirable
