Care Partner - Aged Care at Home

We have a casual opportunity available for a Care Partner supporting our clients within the North Lakes region.
Your Role
As a Care Partner, you’ll be supporting the most important people in our organisation – our clients. You will play a key role in coordinating and delivering client-centred care, working closely with clients, families and internal teams to support independence and wellbeing at home. You will take a holistic approach to care planning and service delivery, ensuring services are responsive, compliant and aligned to each client’s goals. Your tasks will include:
Enabling clients to maintain independence by encouraging choice, autonomy and skill development
Assessing, planning, coordinating and reviewing care delivery to meet individual client needs
Developing and maintaining care plans, client documentation and service records
Monitoring client wellbeing and identifying, documenting and responding to changes in needs or behaviours
Coordinating services and liaising with Allied Health, Nursing and broader teams to deliver holistic care
Supporting scheduling and ensuring the right staff are matched to client requirements
Conducting risk and hazard assessments and supporting safe service delivery
Managing client budgets and service delivery within funding guidelines
Building strong relationships with clients, families and stakeholders
Maintaining accurate and timely documentation in line with compliance requirements
Contributing to continuous improvement initiatives and quality outcomes

A compassionate, client-focused approach with a genuine passion for aged and community care
Experience in community services, aged care or health-related environments
Strong communication and interpersonal skills, with the ability to engage a diverse range of stakeholders
The ability to coordinate care and manage competing priorities effectively
Sound problem-solving skills and the ability to respond to changing client needs
Strong organisational and administrative skills, including experience with documentation and systems
The ability to work both independently and collaboratively within a multidisciplinary team
A proactive, flexible and responsive approach to service delivery
A commitment to safety, compliance and continuous improvement

Role Requirements
Proof of immunity, or willingness to become vaccinated against Influenza (annual vaccination), COVID-19 (minimum 1x dose received), Hepatitis B, Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox) and Pertussis (Whooping Cough)
Current National Police Check (with validity in the past 3 years)
Current (P) PAID NDIS Worker Clearance
Current Queensland driver’s licence
Current First Aid & CPR Certificates
Strong computer and mobile device skills
Relevant qualifications and/or experience in health, aged care or community services (highly desirable)
