Client Liaison - Aged Care at Home

We have a casual opportunity available for a Client Liaison supporting our clients within the North Lakes region.

Your Role

As a Client Liaison you’ll be looking after the most important people in our organisation – our clients. You will be the key point of contact for clients and their families, supporting them to access services, navigate care requirements and maintain independence at home. You will build strong relationships, coordinate services and ensure a positive client experience while promoting a holistic, client-centred approach. Your tasks will include:

  • Conducting intake conversations, service enquiries and initial client assessments in collaboration with the broader team

  • Coordinating and scheduling services to meet client needs while ensuring continuity of care

  • Providing high-quality customer service and acting as the primary contact for client enquiries and support

  • Liaising with clients, families and internal teams to ensure services are delivered in line with individual care plans

  • Monitoring service delivery, identifying changes in client needs and escalating where required

  • Supporting onboarding processes for new clients, including documentation and system updates

  • Maintaining accurate, timely records in accordance with organisational and compliance requirements

  • Assisting with service reviews and contributing to continuous improvement initiatives

  • Responding to general enquiries and supporting administrative functions within the team

Role Requirements

  • Current National Police Check (with validity in the past 3 years)

  • Current (P) PAID NDIS Worker Clearance

  • Good computer and mobile device skills

  • Previous experience in a similar role or experience with administration, customer service, community/aged care is desirable
