As an Aged Care Assessor (Non-clinical), you’ll play a vital role in helping people access the right aged care supports to maintain their independence. Your focus will be on understanding each individual’s unique needs and circumstances, particularly those from diverse and vulnerable backgrounds, including LGBTQI individuals, culturally and linguistically diverse (CALD) communities, and Indigenous populations.

You’ll ensure clients' voices are heard and respected in the care planning process, fostering an inclusive and supportive environment.

What you'll be doing;

  • Conducting person-centred, non-clinical assessments and delegations to identify aged care support needs.
  • Implementing Reablement strategies and Linking Service Support to promote independence.
  • Collaborating with clients, families, and service providers to ensure seamless access to appropriate services.
  • Engaging with the community and contributing to quality improvement initiatives.

To be successful, you will require;

  • Desirable- Certificate IV in Aged Care, Disability, Social Services (or other relevant qualification) OR relevant experience working in the Aged Care, Allied Health, Community, Disability Sector and/or training in in the delivery of aged care assessments.
  • Current Victorian Drivers Licence
