Clinical Nurse Consultant - Aged Care
Bathurst Health Service
About the role
Join the Aged Care Team at Bathurst Health Service as a Clinical Nurse Consultant and help build a new multidisciplinary model focused on supporting older people earlier, coordinating care more effectively to minimise hospital stays.
The aim of the Aged Care Team is to identify healthcare needs earlier, plan care more holistically and support older people before their needs escalate into crisis.
You will be part of complex decision-making, contribute your professional judgement and work across hospital, community, primary care and residential aged-care services. You will work as part of a connected multidisciplinary team where each professional brings its expertise and care is planned together.
As the Clinical Nurse Consultant, you will provide expert consultation to older people, their families and carers and other members of the multi-disciplinary team to achieve the best healthcare outcomes.
