About this opportunity
As a Care Manager, you will be responsible for coordinating and managing care services for prospective and existing clients, ensuring a high-quality and consistent client experience. You will lead the end-to-end client journey including consultation, onboarding, care planning and ongoing reviews, supporting clients to live as independently as possible at home.
Reporting to the Client Experience Manager, you will work closely with caregivers, families and internal teams to ensure services are aligned to client needs, compliant with standards and delivered to a high standard.

Key responsibilities
- Manage and coordinate care for prospective and existing clients across the full journey
- Develop and review individualised care plans aligned to client goals and needs
- Build trusted relationships with clients and families as the key point of contact
- Conduct onboarding, care consultations (when required), assessments and regular care reviews
- Collaborate with internal teams and external providers to deliver holistic care
- Maintain accurate records while ensuring compliance and continuous improvement
About you
You are an organised and client-focused professional who is passionate about delivering high-quality care outcomes. You bring a proactive approach, strong communication skills and the ability to manage multiple priorities in a fast-paced environment.
You'll also have:
- Experience in care management, case management or a similar role
- Understanding of aged care, Home Care Packages/Support at Home or NDIS
- Strong stakeholder and relationship management skills
- High attention to detail and strong administrative capability
- Ability to work both independently and as part of a team
- Relevant qualification in health, social work, nursing or related discipline (preferred)
