We are currently seeking a passionate and experienced professional preferably AHPRA-registered as a Nurse, Occupational Therapist, or Social Worker to join the Melbourne team as a permanent, full-time
Care Manager. This role offers a flexible hybrid working model, including the option to work from home 1–2 days per week.
In this position, you will play a key role in developing and maintaining client relationships while overseeing service compliance for a diverse portfolio of clients across Mount Waverley, Werribee, Essendon, and surrounding regions. Working closely with a dedicated team of Care Staff, you will ensure care plans are effectively implemented and continually adapted to meet the unique and evolving needs of each client.
Key duties and responsibilities will include (but not be limited to):
- Coordinate and attend meetings with prospective clients on a weekly basis
- Process client assessments and develop personalised care plans tailored to individual needs
- Oversee the smooth onboarding of new clients into care services
- Manage an ongoing portfolio of clients, adapting care plans as required and handling any incidents promptly and professionally
- Consistently review client progress notes and ensure corrective actions are implemented
- Build and maintain strong, respectful relationships with clients and their families
- Gather and utilise client and family feedback to enhance service quality and internal processes
- Coordinate care staff to ensure clients receive time and appropriate services, working closely with the scheduling team
- Chair regular team meetings with staff to support them in their day-to-day duties
- Lead and support your care team through regular performance reviews, roster management and team meetings
Skills And Experience Required For This Position Will Include
- Working experience as an AHPRA registered professional (Nurse, Occupational Therapist or Social Worker)
- Demonstrated leadership skills with proven ability to manage a large team and day-to-day changes efficiently
- A compassionate and empathetic approach to supporting aged care clients
- Strong organisational skills with the ability to manage competing priorities
- Reliable and self-motivated, with a commitment to delivering high-quality care
- Collaborative team player who thrives in a supportive and dynamic environment
- Excellent communication skills with the ability to build trust with clients and families
- Confident in providing professional advice and guidance tailored to individual client needs
- A proactive problem-solver who remains calm and focused under pressure
Remuneration
This position offers a competitive salary package commensurate with experience managing Home Care Packages. In addition, the successful candidate will enjoy a range of benefits including a fully serviced vehicle and fuel card (with reasonable personal use permitted), hybrid working flexibility (1–2 days per week from home), and essential tools such as a company laptop, mobile phone, and headset. Performance-based bonuses are also paid on a 6-monthly basis and can significantly increase your annual earnings, recognising and rewarding your contributions to the team’s success.
