About the role

Anglicare is looking for a Physiotherapist to work in the Wide Bay region with community aged care clients.

Responsibilities of the role include the following:

  • Manage internal and external referrals for physiotherapy services.
  • Complete client assessments on admission to physiotherapy.
  • Determine appropriate goals and develop the collaborative plan of care to meet these goals.
  • Select and provide appropriate treatment and advice to clients and carers.
  • Obtain appropriate equipment as required by liaising with appropriate agencies.
  • Refer to other services when required to meet the client’s needs.
  • Encourage rehabilitation and reablement by assisting the client and carers towards self-care and maximum independence.
  • Work effectively in multidisciplinary teams to maximise client outcomes, including attending case conferences as required to advise on client care, assessment results and progress.
  • Ensure all physiotherapy services reflect best practice, including evidence based practice

Requirements:

  • Current National Police Certificate
  • Current Drivers Licence with own reliable vehicle
  • Degree in Physiotherapy
  • Current registration with AHPRA
  • First Aid and CPR Certification

About you:

  • Demonstrated clinical knowledge and ability to competently assess and treat aged care clients in a community setting.
  • Demonstrated ability to develop, implement and review Physiotherapy services.
  • Demonstrated ability to work collaboratively with clients and their families and/or carers to maximise client outcomes.
  • Demonstrated ability to work within a community based multidisciplinary team environment, including the capacity to negotiate with a range of stakeholders, operate flexibly, and have the capacity to problem solve.
  • Ability to work autonomously and plan and prioritise work effectively.
  • Demonstrated well developed oral, written and interpersonal communication skills.
  • Demonstrated understanding of quality management principles in the
