As a Registered Nurse in Community Care Coordination, you’ll play a key role in ensuring individuals receive the right care at the right time. Working within a collaborative multidisciplinary framework, you’ll lead comprehensive assessments, coordinate tailored care plans, and connect consumers with the services and supports they need to thrive in the community. This is a fast-moving, highly engaging role where your clinical expertise, communication skills, and ability to navigate complex care pathways will directly impact outcomes. You’ll build strong partnerships with consumers, families, and service providers, all while driving continuity of care, supporting recovery journeys, and making a tangible difference across the mental health system.

From Helensburgh in the north to North Durras in the south, ISLHD covers an area of 5,687 square kms. That’s 5,687 square kms of beautiful beaches, coastal cycling tracks, lush rainforests, incredible bushwalks and friendly people.
