Position Overview

Blue Sky Community Services is seeking a professional and values-aligned person to join our supportive and friendly team. In this role you will help deliver My Aged Care Assessment Services across the Coffs Harbour region and surrounding areas. This is a remote position. This role is under the SCHADS Award Level 5.1 $51.00 – Level 5.3 $53.31 per hour.

Blue Sky Community Services is delivering the Aged Care Single Assessment Service in partnership with My Aged Care and Community Options Australia.

Aged Care Assessors:

  • Provide person-centred, comprehensive, timely, and high-quality assessments that support participants to access Commonwealth-funded aged care services and achieve meaningful outcomes
  • Apply a wellness and reablement approach to enable participants to remain as independent as possible in their own homes
  • Deliver short-term linking support to assist participants to identify, access, and engage appropriate services to meet their care needs
  • Manage an allocated caseload, including prioritising referrals and meeting required service timeframes
  • Maintain accurate, timely, and compliant documentation within My Aged Care systems and organisational databases
  • Identify risks and safeguarding concerns (including vulnerability, neglect, or abuse) and escalate in line with organisational procedures
  • Collaborate with internal teams, service providers, health professionals, and other stakeholders to ensure coordinated and effective care outcomes
  • Deliver culturally safe, inclusive, and responsive assessments for people from diverse backgrounds and communities
  • Promote and clearly communicate the Aged Care Assessment process, its purpose, and outcomes to participants and the community
  • Support the community to build understanding of the aged care system and available services
  • Ensure compliance with relevant legislation, guidelines, organisational policies, and work health and safety requirements

Who We Are Looking For

The successful applicant will have:

  • Tertiary qualifications in a health-related discipline directly relevant to aged care or a specialised field (e.g. Registered Nurse, Medical Officer, Occupational Therapist, Social Worker)
  • Willingness to travel for face-to-face assessments, with access to a fully registered and insured vehicle for work purposes, including roadside assistance
  • Current, unrestricted registration with the Australian Health Practitioner Regulation Agency (AHPRA), or membership (or eligibility for membership) of a relevant professional association
  • Evidence of current COVID-19 and influenza vaccinations
  • The ability to consistently demonstrate behaviours, attitudes, and conduct aligned with the organisation’s values, culture and code of conduct
  • A strong working knowledge of the aged care system, including services, the assessment process, and associated tools
  • A genuine passion for delivering exceptional customer service
  • The ability to quickly learn and confidently use information and communication technology, including Microsoft Office, client management systems, and data bases
  • The capability and willingness to confidently promote and accurately represent the Aged Care Assessment process, its purpose, and functions

Please Note

A willingness to travel within the region is essential. Applicants will need to have their own fully insured and registered vehicle available for work purposes, with Roadside Assistance.

Applicants Will Also Be Required To Provide Evidence Of

  • Tertiary qualifications in a health-related discipline related to health, aged care or related specialist area. E.g. Registered nurses, medical officers, occupational therapists, social workers
  • Current registration with Australian Health Practitioners Regulation Agency (AHPRA) or part of or eligible to be part of a relevant professional association
  • Evidence of current vaccination against COVID19 and influenza

About The Aged Care Assessment Team

Blue Sky Community Services is providing aged care assessment services in the Mid North Coast, Far North Coast and New England regions, supporting many older Australians.

The Recruitment Process

Stage One: Application Assessment Stage

  • At the interview, applicants will be asked questions to determine whether they are able to perform to a high standard within the role and to determine whether they are a good fit for the position, the organisation and our values.

Stage Three: Further Information Gathering Stage

  • Following the interview, applicants may be asked to complete online psychometric testing to further determine suitability for the position.
  • Following the interview, the organisation may contact the applicant’s referees to further determine suitability for the position.
  • Applicants may also be requested to attend a second interview.

Stage Four: Employment Stage

  • Successful applicants will be contacted by the organisation to discuss employment possibilities.
