We are seeking a collaborative team member to join ITACS who is motivated to continuously improve processes, demonstrates strong attention to detail, and brings excellent communication skills.

In this role, you will engage in daily communication with a wide multidisciplinary team, supported by the ITACS Management team. You will play a key role in strengthening communication pathways with referring teams to support timely discharge coordination, accurate completion of documentation, and effective information gathering.

As a primary point of contact for the ITACS service, you will also provide patients and their families with clear information about the program and support initial care planning, helping to ensure a smooth and coordinated entry into services.

A little about us.

As a part of the new Shellharbour Hospital and Integrated Services Project, a new Community Health Centre is being built on the old Port Kembla Hospital site. The Warrawong Community Health Centre will bring together a range of services including community mental health, drug and alcohol, community nursing, and child and family services.

Until the new Community Centre is complete, some allied health services will continue to operate out of the Port Kembla Hospital site.
