About the Role
The Community Based Chronic Disease Palliative Care Team provides essential allied health services to palliative care patients in the community who have late stage degenerative and chronic conditions and progressive disability, specifically, COPD, CHF, Dementia and Neurodegenerative Diseases, spanning the last two years of life. This model of care provides much needed allied health support to enhance patients’ quality of life and support to remain at home as long as possible.
Provide senior clinical leadership within the Sydney Virtual Hospital, Integrated Chronic Care, palliative care chronic disease, Physiotherapy service provision, including optimisation of patient/client care delivery, as well as supporting the mentoring and development of Physiotherapy staff and students. This is an Sydney Virtual Hospital position, providing physiotherapy care to palliative care patients with a chronic disease, in the community setting.

Ideal Candidate
- Current AHPRA Physiotherapy Registration to practice as a physiotherapist, degree, or equivalent in physiotherapy and eligibility for membership of the Australian Physiotherapy Association with experience at Level 2 or higher.
- Advanced clinical knowledge, skill, and significant clinical experience in the provision of clinical care across a wide range of practice areas within their discipline including palliative care.
- Ability to independently problem solve, apply professional and ethical boundaries when working with complex situations as per of a multidisciplinary team.
- Must hold a current NSW driver licence (P1 and P2 acceptable).
