An exciting opportunity exists for a motivated and compassionate Social Worker to join the Social Work Department at Bundaberg Hospital, supporting the Frailty Virtual service. This role offers the chance to work within a multidisciplinary team delivering patient-centred care to older adults through innovative virtual healthcare models. Applications are invited from suitably qualified Social Workers who are passionate about improving the lives of older people through innovative models of care.

Your Opportunity

  • To deliver high level social work clinical services and expertise to patients identified for the rapid access geriatrician-led model of care, as a multidisciplinary team member.
  • This short sub-acute service (up to 14 days) approach aims to reduce prolonged in-hospital patient admissions (extended stays). The position will facilitate a safe discharge and monitoring so that patients can continue living at home while improving their functioning and connecting with care in the community.
  • This position will focus on preventing inappropriate or avoidable admissions by supporting the needs of the patient, their family, and significant others through sub-acute supports in the home environment, to optimise their psychosocial outcomes and achieve maximum benefits from health care services.
  • This position reports to the Director of Social Work.

Qualifications and other requirements

  • Mandatory possession of a recognised degree in Social Work, and eligibility for membership with the Australian Association of Social Workers (AASW). Eligibility is dependent on the candidate having completed a University level Social Work degree accredited by the AASW and that the candidate does not have any other outstanding professional competency or ethical issues or complaints known to the AASW which may preclude membership.
  • Overseas qualified Social Work candidates require a “certification of eligibility” letter from the AASW to confirm their eligibility for membership. Overseas qualified applicants should include such evidence with their application to be considered for short-listing in the selection process.
  • Appointment to this position requires proof of qualification and if applicable registration or membership with the appropriate registration authority or association. Certified copies of the required information must be provided to the appropriate supervisor/manager, prior to the commencement of clinical duties.
  • CPE accreditation and previous experience in an acute hospital setting is highly desirable.

Our offer: You will receive a competitive remuneration package value up to $39,770 p.a., comprising salary rates $39.87 - $58.60 p.h., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP3) (Permanent Part Time, 22.8 hours p.f.).
