Position Overview

The Clinical Care Partner (RN) provides clinical oversight and expert guidance across Home Care services, ensuring safe, high-quality, person-centred care aligned with the Aged Care Act 2024 and Support at Home Program. Working closely with Service Managers, Care Partners, and site-based clinicians, this role supports care planning, clinical governance, and outcomes that promote participant autonomy, dignity, and wellbeing. This role plays a critical part in maintaining clinical governance and supporting compliance with the Aged Care Act 2024, the Support at Home Program Manual, and the Strengthened Aged Care Quality Standards

Position Responsibilities

  • Provide clinical oversight and guidance across the Home Care services, ensuring alignment with participant goals, assessed needs, and the principles of the Aged Care Act 2024.
  • Collaborate with Care Partners to review and support the development of individualised and culturally appropriate participant care plans, offering clinical advice and recommendations for complex care needs, and ensuring care plans identify wellness and reablement approaches to meeting participant goals.
  • Deliver direct clinical care services, ensuring care is evidence-based, person-centred, and aligned with best practice standards. This includes undertaking clinical assessments, providing wound care, medication management, and other nursing interventions as required. Direct care delivery will complement broader clinical oversight responsibilities, support continuity of care and enhance clinical outcomes.
  • Monitor and review reported clinical incidents, ensuring appropriate escalation, documentation, and follow-up in accordance with the Serious Incident Response Scheme (SIRS).
  • Delegate clinical tasks to maximise labour efficiencies and utilisation of site-based Registered/Enrolled Nurses, ensuring delegation aligns with scope of practice, clinical governance, and participant safety.
  • Support clinical decision-making by interpreting assessments, identifying risks, and advising on appropriate interventions or referrals.
  • Promote participant autonomy and dignity by supporting informed decision-making and ensuring care aligns with the Strengthened Aged Care Quality Standards.
  • Liaise with Service Managers and site-based clinical teams to ensure consistent, high-quality care delivery and adherence to clinical protocols.
  • Participate in multidisciplinary case reviews and care conferences to ensure coordinated, person-centred care planning.
  • Contribute to continuous quality improvement by participating in audits, incident reviews, and feedback processes, including evaluation of responses or actions to ensure effective outcomes.
  • Maintain accurate and timely clinical documentation in line with organisational and legislative requirements.
  • Provide clinical leadership and mentoring to site-based Nurses, supporting professional development and best practice.

Key Selection Criteria

SC1: Qualifications and Knowledge

Essential:

  • Bachelor of Nursing
  • Current AHPRA registration as a Registered Nurse
  • Current First Aid and CPR certificates
  • Current driver’s licence

Desirable:

  • Postgraduate qualification in Gerontology, Dementia, or Aged Care

SC2: Experience (Desirable)

  • Minimum 12 months relevant experience
  • Home and Community Nursing experience with care planning knowledge
  • Experience using mobile devices for documentation and care management

SC3: Technical Competence

  • Intermediate computer skills (Word, Excel, Outlook, PowerPoint)

SC4: Personal History Checks

  • National Police Certificate (or ability to obtain)
  • NDIS Worker Screening Check (where applicable)
  • National Personal Insolvency Check

SC5: Responsible Person – Aged Care

This position is formally designated as a Responsible Person under the Aged Care Act 2024 (Cth).

As a Responsible Person, you are legally accountable for ensuring the organisation complies with its obligations under the Act.
