About The Role
This is a part-time, specialist role (1 day per week – Wednesdays) working as an independent practitioner to support a defined cohort of ophthalmology patients with diabetes.
The Diabetes Clinical Nurse Consultant will focus on improving patient engagement, self-management, and attendance at scheduled appointments and procedures within Specialist Clinics. The role is central to optimising patient outcomes by supporting individuals to better manage their diabetes and associated eye disease, reducing avoidable delays in care.
Working closely with ophthalmology, nursing, and allied health teams, you will provide targeted education, coordination, and follow-up for patients at risk of non-attendance or poor disease control.
Key Responsibilities
- Provide high-quality diabetes education, assessment, and support to patients and carers in the non-surgical pathways program.
- Collaborate with multidisciplinary teams to develop and monitor individualized care plans from admission to discharge.
- Contribute to effective discharge planning and refer patients to ongoing external diabetes services as needed.
- Act as a clinical resource, educating staff and patients on best-practice diabetes care, medication management, and self-management techniques.
- Participate in quality improvement activities, data collection, and reporting to demonstrate program outcomes.
- Ensure compliance with hospital policies, privacy legislation, and safety standards.
- Actively engage in professional development and maintain a CPD portfolio.
About you
Selection Criteria
Essential
- Division 1 Registered Nurse with current registration with AHPRA (Victoria)
- Post-graduate Certificate in Diabetes Education
- Credentialed Diabetes Educator (ADEA CDE) status
- Minimum 5 years post-registration experience
- Ability to work independently and within interdisciplinary teams
- High level of diabetes knowledge, skills, and clinical competence
- Excellent written and verbal communication skills
- Self-motivated, able to plan and organize own workload
Desirable
- Experience with Patient Management Systems/Digital Health Record systems (e.g., BOSSNET, Cerner, Pims)
- Experience with hospital reporting systems (e.g., Riskman/VHIMS)
- Experience in providing formal and informal education for nursing staff
Competencies
- Basic Life Support (as per hospital’s mandatory training framework)
