About The Role

You will work closely with clients, carers and interdisciplinary teams to deliver evidence-based occupational therapy services that improve wellbeing, independence and quality of life.

Key responsibilities

  • Conduct comprehensive OT assessments and develop client-centred care plans
  • Complete home visits and recommend assistive technology, equipment and home modifications
  • Work with funding through My Aged Care, CHSP, HACC PYP, Support at Home, SWEP and Geat2Go
  • Provide coaching, education and self-management support to clients and carers
  • Collaborate with Physiotherapists, Podiatrists, Allied Health Assistants and other clinicians
  • Participate in supervision, case reviews and quality improvement initiatives
  • Maintain accurate documentation and contribute to service KPIs

About You

You are a compassionate and motivated Occupational Therapist who values teamwork, continuous learning and community health.

You will bring

  • Current AHPRA registration (or eligibility)
  • Experience or knowledge in functional assessments, home modifications and assistive technology
  • Strong communication, organisation and problem-solving skills
  • Ability to work effectively with diverse and CALD communities
  • Commitment to evidence-based practice and high-quality client care
