About The Role

The Client Liaison Coordinator plays a key role in guiding customers through the NDIS and HCP processes and connecting them to the right services within the organisation.

As the first point of contact, this role helps customers achieve their funded goals by coordinating their services and ensuring a smooth experience. The Client Liaison Coordinator is crucial to providing excellent service, supporting client retention, and helping the organisation grow. By assessing client needs and ensuring services align with care plans and funding, this role promotes client independence and well-being.

Key Responsibilities

  • Coordinate client intake, including assessments, goal setting and preparation of service agreements
  • Develop, review and maintain personalised care plans aligned with funding requirements
  • Serve as the primary contact for clients, families and carers, ensuring clear and timely communication
  • Coordinate and schedule services across internal teams and external providers
  • Monitor client satisfaction, address concerns promptly and support continuous service improvement
  • Maintain accurate and compliant documentation, including care plans, agreements and progress notes
  • Review client plans to ensure alignment with goals and effective use of funding
  • Build strong, collaborative relationships with clients, internal teams and service partners
  • Identify opportunities to enhance service delivery and contribute to ongoing quality improvement initiatives

Essential

  • Knowledge of NDIS and/or Home Care Packages
  • Experience working in community, disability, or aged care services
  • Understanding of CMS or client management systems
  • Ability to manager competing priorities and meet deadlines within tight timeframes
  • Good attention to details with ability to build and maintain relationship with internal and external stakeholders
  • Focus on continuous improvement to improve the customer experience

Compliance

  • NDIS Worker Screening Check.
  • NDIS Orientation Certificate
  • Participate in relevant trainings
