As a Care Consultant at Baptistcare, you won’t be processing people through a system. Instead, you’ll be walking alongside them, helping them stay safely and independently in the homes they love.

THE ROLE

You’ll be the primary point of contact for a caseload of home care customers assessing their needs, planning their services, managing their budgets, and visiting them regularly to make sure their care is actually working for them.

Conduct intake, initial, and ongoing assessments of aged care-related needs

Develop and maintain goal-directed care plans within each customer’s home care budget

Coordinate and facilitate supports and services across the funded program

Manage package spend and advocate for customers’ needs and preferences

Conduct regular home visits and ongoing monitoring to support customers to age well in place

Build strong relationships with customers, families, and community stakeholders

ABOUT YOU

You bring a mix of clinical or case management foundations, genuine empathy, and the organisational skills to manage a complex caseload independently.

Qualification in Case Management, or an undergraduate qualification in Nursing, Allied Health, or Social Work (essential)

Prior case management experience, particularly in home care or community services (highly regarded)

Experience completing initial and annual customer assessments and care planning

Knowledge of the Support at Home program and the broader home care sector

Division 1 Nursing registration (desirable but not essential)

Based in Melbourne’s Northern Suburbs with a current driver’s licence and willingness to travel across the region

National Police Check (or willingness to obtain)

Proof of working rights in Australia
