The Clinical Nurse Specialist Grade 2 (CNS provides expert clinical leadership and coordination of patient care within the Hospital@Home (H@H) service to support safe, effective and timely care delivery. The role supports nursing staff and the multidisciplinary team through the provision of advanced clinical advice, coordination of daily clinical activity, and facilitation of communication to optimise patient flow and minimise clinical risk.

The CNS2 contributes to service efficiency by monitoring daily workloads, coordinating available resources within agreed models of care, participating in daily virtual ward rounds, and escalating clinical or capacity concerns as required to support safe patient outcomes and continuity of care.

The Hospital@Home service is provided from multiple sites and therefore a requirement of the position will be to work across the District as required.

The hospital has an Intensive Care Unit focusing on high dependency and coronary patients, Children’s Ward and Maternity Unit, as well as a purpose-built regional Cancer Care Centre.

The hospital is currently undergoing a significant $440M redevelopment that will redefine how healthcare is delivered within the Region.
