We have an opportunity in Mildura, VIC for Home Care Team Leader to join our dynamic team.

What does the Home Care Team Leader do?
The Home Care Team Leader works within the In-Home Support team providing services to our customers and guidance and support to our Home Care Workers. The role assists customers to maintain their independence in a community setting and provides episodic and ongoing domestic assistance, personal care, shopping, respite, transport, and social support to people living in the community. The Home Care Team Leader is required to undertake rostered services with our in-Home Support Services customers as well as provide daily support and supervision to our team of Home Care Workers. They are also required to undertake and pass LiveBetter Clinical Care Core Competencies training and other mandatory training.
Some key accountabilities of the role will include:
- Providing guidance and support to new and existing Home Care Workers in the field.
- Daily supervision and leadership of staff, including monitoring staff compliance.
- Onboarding, orientation and on the job training with “Buddies” for new Home Care Workers.
- Assisting Home Care Workers with managing challenging situations, improving customer satisfaction with guidance and support to meet care needs.
- Management of Home Care Worker Competency Assessment and sign off including Quality Support Visits.
- Provide feedback to the Area Manager and Coordinator regarding in field performance of Home Care Workers if required.
- In collaboration with the In-Home Support Team, initiate team meetings and one-on-one catch ups with Home Care Workers when required.
- Providing additional support for rostered services with High-Needs customers that might require 2:1 transfers or behaviours that require a more experienced staff member.
- Office based administration work as and when required.
What does LiveBetter require from you?
- Certificate III in Aged Care, Individual Support or equivalent.
- Proven experience and high levels of work quality as a Home Care Worker/Support Worker or Similar
- Ability to lead, mentor and train home care workers in the field.
- High-level of computer literacy and ability to manage multiple systems
- A caring and compassionate approach with the ability to communicate effectively.
- Ability to work effectively in a team environment as well as independently and working alongside Community Care Coordinator/Program Coordinator to raise the standards of our care delivered in the community.
- Experience in the use of technology such as smart phones and computer programs.
